Insurance and cost

Using insurance for counseling, explained plainly

Brett is in network with four carriers across Ohio and Kentucky, so most clients pay a copay and nothing more. If your plan is not one of them, private pay is $150 a session and out-of-network reimbursement is often possible. Start by picking your plan below, then see the cost.

Step one

Which insurance do you have?

Pick your plan and we will show you what to expect. Brett is in network with four carriers across Ohio and Kentucky and confirms your specific plan before the first session, so there are no surprises.

What to expect

Choose a plan to see what you would pay

In-network visits usually cost a copay or a share of the fee until your deductible is met. Private pay is $150 a session and always available.

UnitedHealthcare

In network

One of the largest insurers in Ohio and Kentucky. Most UnitedHealthcare employer and individual plans route mental health benefits through Optum, so a UHC card and an Optum card usually mean the same network.

Plans Brett sees

  • Employer plans (Choice, Choice Plus, Options PPO, Navigate)
  • Individual and family plans
  • UnitedHealthcare Community Plan (Medicaid) and Medicare Advantage vary by product; ask on the call

On your card

Look for "UnitedHealthcare" on the front and, often, "Optum" or "United Behavioral Health" for mental health on the back.

Four questions for the number on your card

  1. 1.Call the member services number on the back of your card and ask for behavioral health benefits.
  2. 2.Ask: is Brett C. Larsen, LPCC-S, in network for outpatient mental health visits under my plan?
  3. 3.Ask what you pay per visit: a copay, or coinsurance after a deductible, and how much of the deductible is left.
  4. 4.Ask whether video visits are covered the same as office visits.

Video sessions: UnitedHealthcare generally covers video counseling with an in-network provider the same way it covers an office visit. Confirm for your plan.

Optum

Optum

In network

Optum manages behavioral health for most UnitedHealthcare plans and for some employers directly. If your benefits card, welcome email, or employee assistance program mentions Optum, United Behavioral Health, or Live and Work Well, this is your network.

Plans Brett sees

  • Behavioral health for UnitedHealthcare commercial plans
  • Employer behavioral health carve-outs administered by Optum
  • Some employee assistance programs (EAP) that use the Optum network

On your card

Your medical card may say UnitedHealthcare while the behavioral health line on the back says Optum. Either way, use the behavioral health number.

Video sessions: Optum covers video visits with in-network providers under most plans. Confirm whether your plan pays telehealth at the office-visit rate.

Anthem Blue Cross and Blue Shield

In network

Anthem is the Blue Cross and Blue Shield licensee for both Ohio and Kentucky, which is why Brett is in network for clients on either side of the river. Anthem sells employer, individual, and Medicaid plans in both states.

Plans Brett sees

  • Employer plans (Blue Access, Blue Preferred, PPO and HMO products)
  • Individual and family Marketplace plans
  • Anthem Ohio Medicaid and Kentucky Medicaid managed care; benefits differ, ask on the call

On your card

Look for the Blue Cross and Blue Shield cross and shield with "Anthem." Cards from other Blue plans carry a suitcase logo; those may pay through the BlueCard program, so ask.

Video sessions: Anthem plans in Ohio and Kentucky generally cover video counseling with in-network providers. Some plans set a different copay for telehealth, so ask.

CareSource

In network

A Dayton-based nonprofit health plan that serves Medicaid members in Ohio and Kentucky and sells Marketplace plans in both states. CareSource members do not need a referral to see a counselor.

Plans Brett sees

  • Ohio Medicaid managed care
  • Kentucky Medicaid managed care
  • CareSource Marketplace individual plans in Ohio and Kentucky

On your card

CareSource cards say which program you are in. Medicaid and Marketplace benefits work differently, so tell Brett which one you have.

Four questions for the number on your card

  1. 1.Call CareSource member services on your card and ask for behavioral health benefits.
  2. 2.Ask whether Brett C. Larsen, LPCC-S, is in network for outpatient counseling under your plan.
  3. 3.Medicaid members usually have no copay for counseling. Marketplace members should ask about copay or coinsurance and deductible.
  4. 4.Ask whether video visits are covered the same as office visits.

Video sessions: CareSource covers telehealth counseling for Medicaid and Marketplace members. Video sessions with Brett run on Carepatron, not on the Teladoc line CareSource advertises.

Aetna

In network

Brett uses Headway, a third-party service, to submit claims to Aetna. To use your Aetna insurance for sessions, you sign up for Headway, add your insurance details, and confirm your coverage and session cost. Brett is happy to walk you through it during your free consultation.

Plans Brett sees

  • Employer plans (Aetna Choice POS II, Open Access, HMO)
  • Individual plans through Aetna CVS Health
  • Medicare Advantage varies; ask on the call

On your card

Aetna cards list the plan type on the front. A POS or PPO plan is the most likely to include out-of-network benefits if in-network does not apply.

Video sessions: Aetna generally covers video counseling. Confirm the rate for your plan.

Coverage depends on your specific plan, not just the carrier. Tell Brett your plan on the free 10-minute call and he will confirm what applies before you book. Source for in-network status: the practice's own site and Brett's Psychology Today profile, checked 2026-09-12.

Step two

How billing works with Brett

  1. 01

    Tell Brett your plan

On the free 10-minute call, read the carrier and plan name off your card. Brett confirms whether it is in network before you book anything.

  2. 02

    Book the initial assessment

Your first full session. If you are in network, Brett bills the insurer directly after the visit. Private-pay clients settle at the session.

  3. 03

    Pay only your share

In network, you pay the copay or coinsurance your plan sets, and nothing else. Brett handles the claim.

  4. 04

    Keep going without paperwork

Each session is billed the same way. If your plan changes at renewal, tell Brett and he checks again.

Not in network?

You can still work with Brett. Pay the private-pay fee at the session and ask for a superbill. PPO and POS plans often reimburse a portion of out-of-network counseling once your deductible is met. HMO and EPO plans usually do not. Your insurer can tell you which you have.

Five questions to ask your insurer

  1. 1.Is Brett C. Larsen, LPCC-S, in network for outpatient mental health under my plan?
  2. 2.What is my copay for an outpatient counseling visit, CPT 90837 or 90834?
  3. 3.Do I have a deductible for mental health, and how much of it is left this year?
  4. 4.Are video visits covered at the same rate as office visits?
  5. 5.Is there a limit on the number of sessions per year?
